To Request Packages:

Please complete the form below and submit.  The pop-up “thank you” confirms we have received your request.  Note: Due to our production cycle, we cannot guarantee shipment or delivery dates.

If you are submitting more than five names at the same address, do not use this Individual Request Form.  Instead, please use this Group Request Form or email [email protected] to ask for the Group Request Form.

-Rank will NOT be included on the mailing label.
-We will accept requests for individuals from service members or immediate family only.
-Due to security and confidentiality concerns, we can only accept lists of names from a military member of the deployed unit.  The “Submitter” email in Column J of the Group Request Form needs to be of a military POC (Point of Contact) deployed with the unit.
-As we are trying to support as many troops as possible, only one request per service member will be accepted during a six month period.
-While we would like to support every American service member anywhere in the world, due to limited resources, we only send packages to service members deployed: overseas in hostile environments or on unaccompanied hardship tours or at sea.  Thank you for your understanding.

Your submission of information is completely confidential. Operation Gratitude will not release names, addresses or emails of troops or submitters. Email addresses may be used for Operation Gratitude correspondence. All letters and packages will arrive directly from Operation Gratitude Headquarters, postmarked Van Nuys, CA.

If your APO or FPO includes 9 digits, include ALL 9 digits. Do not include any commas (,).
