Individual Request Form

Request Packages:

Based on our shipping schedules and to avoid returns, we can only accept package requests for deployed troops who will return from combat zones 
after August 1, 2014

Instead, please use this Group Request Form or email to ask for the Group Request Form or email the names to: OpGrat@aol.com as an attached Excel file as per this sample; please include all columns, even if blank, and type the first line of the address in the subject line of your cover email.

-Rank will NOT be included on the mailing label.
-Due to security and confidentiality concerns, we can only accept lists of names from a military member of the deployed unit; we will accept requests for individuals from service members or immediate family only.
-As we are trying to support as many troops as possible, only one request per service member will be accepted.
-While we would like to support every American service member anywhere in the world, due to limited resources, we only send packages to troops deployed in combat environments who receive combat duty pay.  Thank you for your understanding.

Wounded Warrior Care Package Program: We provide special packages to hospitals and WW Transition Units.  To request, please contact cblashek@gmail.com
Battalion Buddy Program: We send special stuffed animals to the children of deploying/deployed troops upon request of the Family Readiness Coordinator.    For more information or to request, email: BattalionBuddy@gmail.com

Your submission of information is completely confidential. Operation Gratitude will not release names, addresses or emails of troops or submitters. Email addresses may be used for Operation Gratitude correspondence. All letters and packages will arrive directly from Operation Gratitude Headquarters, postmarked Van Nuys, CA.
